(DHS) being the entity that operates Oldham Juniors Rugby Club, may collect, use and disclose the personal data (including all personal data as provided in this form) for the following purposes in accordance with the Personal Data Protection Act 2012: (a) the processing of this registration; and (b) the administration of your child/ward’s participation in the relevant competitions/ tournaments and all other activities organised by us; and (c) the promotion of our club through the use of any photographs, video and/or any other imagery

Release & Discharge*
I fully understand and am aware that Rugby is a contact sport and accidents can happen causing property loss/ damage, injury or death and I accept the risk of having my child/ ward participate in the activities organised by Dauntless Hero Sports Pte. Ltd. (DHS) being the entity that operates Oldham Juniors Rugby ClubI hereby release, waive and discharge DHS, including but not limited to, its representatives, coaches and volunteers from, and promise not to sue anyone of them for, any and all liability, claims, demands, actions and causes of action whatsoever arising out of or in relation to any loss, damage or injury including death, that may be sustained by my child/ ward or to any property owned by the same, howsoever caused by participating in the activities organised by DHS.I hereby indemnify and hold harmless DHS, including but not limited to, its representatives, coaches and volunteers and each of the individually from any losses, claims, damages, liabilities, costs and expenses including legal fees that they suffer or incur arising out of or in relation to, directly or indirectly, my child’s/ ward’s presence and/ or behaviour at the activities organised by DHS.I expressly agree that the foregoing release, waiver, discharge, promise and indemnity is intended to be as broad and inclusive as permitted by law and that if any portion is held to be invalid, I agree that the remainder shall continue in full force and effect for so long as my child/ward participates in the activities organised by DHS.
